TELL カウンセリング
サービス内容 TELLカウンセリングでは、臨床心理学、精神科、ソーシャルワーク、カップル・家族療法などを専門とする専門家によるカウンセリング及び心理アセスメントを提供しています。 ご予約までの流れ： ステップ① 問診票をメール又はFAXでご送信 クライエント様のニーズ（成人用、大人用、遠距離用）に合った問診票を右側のリンクからダウンロードし、ご記入の上、Eメールへの添付ファイルとしてご送信をお願い致します。Eメールの宛先は intakesubmission@telljp.comです。なおOS XまたはiOsのご利用の方はPDFとしてご送信頂きますようにお願い致します。 ご予約が可能である時間は平日の通常営業時間と夜の場合もあり、週末はセラピストの都合によります。 ステップ② セラピストからの連絡 提出された問診票をもとに本クリニックのディレクターがクライエントのニーズに応えられるセラピストを推薦し、そのセラピストは直接にクライエントにご連絡致します。子どもの場合は、初回面接が保護者のみとなり、情報収集の75分間の面接となります。 ステップ③ ご予約確認 セラピストに連絡された際、遅くても予約日時の一日（24時間）までにご返信を頂きますようお願い致します。ご予約日の前日を過ぎた際はご予約を保証できません。 ステップ④ ご予約当日 スライディングスケールをご希望される方は、予約日までにintakesubmission@telljp.comまでのご連絡が不可能である場合、ご家庭の収入が証明される書類（ご給料明細や源泉徴収表など）をご持参いただきますようお願い致します。面接のためにご来談される際は待合スペースでご待機ください。セラピストは待合スペースまでお迎えします。 当団体の心理カウンセリングサービスの特徴： 複数言語で対応可。 相談内容は秘密厳守。 個人、カップル又は家族カウンセリングを提供。 子ども向けのカウンセリングは児童から10代までの子どもたちの対応可。 料金はスライディングスケール形式（年収によって料金が段階的に変わる）を採用しており、各クライアントの家庭収入に応じて決定されます。 相談内容：うつ病、パニック障害、不安障害、摂食障害、アルコールや薬物依存、喪失、トラウマ、人間関係、異文化への適応問題、親子関係、子どもの問題行動、引きこもり、思春期、反抗期、不登校など。 場所：東京南青山、横浜市、沖縄の3ヵ所で提供。 アポイントメントは平日及び土曜日、朝9時～夜20時まで予約可。 面接はすべて予約制で行われており、同日の予約はございません。 心理アセスメント （テスティング）サービスの特徴：…
